We request that you notify the office at least 24 hours in advance if you will be unable to keep an appointment. There will be a $50-$100 fee for NOT notifying our office of a cancellation.  We understand that your time is valuable to you and we will always attempt to see you promptly and at the scheduled time. Please assist us by being on time for your appointments. Should an emergency occur which requires a change in the schedule, we will notify you in advance if possible.

Helpful Information:

  • Be aware that your initial visit may take at least an hour. Our patient  information forms have several pages and may be time consuming to fill out. To help speed up this process we have made them available on this website so they can be completed ahead of time.If your forms have been completed and you have copies of your reports we ask that you arrive 15 minutes before your appointment. If you are going to complete your forms upon arrival to the office or we are missing pieces of your information please arrive 30-45 minutes early.
  • It is very important that we see ALL of the mammograms or ultrasound films that have been made. Patients that arrive without their mammorgram films will be re-scheduled! Reports are not enough for us to give you our best consultation. We ask that you personally pick up your films and bring them with you. For your convenience call the imaging center a few days ahead so they have a chance to get things together for you to pick up. Make sure they give you all of your films not just selected images.
  • During your evaluation you will be asked to undress from the waist up. You will be provided a comfortable cloth or paper wrap to wear and your examination will be performed with the highest degree of professionalism. You may wish to dress with this in mind.


The financial responsibility for services rendered rests totally with the patient or responsible party, regardless of insurance coverage. All co-pays, deductibles, and/or co-insurance are expected to be paid at the time services are rendered. All quotes for procedures/surgeries are estimates based on your specific insurance and may be subject to change. These quotes are expected to be paid in full prior to the procedure/surgery. If for some reason you are unable to meet the financial requirements, please let us know in advance and we will be happy to work out a payment schedule with you prior to your appointment or scheduled procedure.


We no longer accept the following insurance: Pyramid Life/Today’s Choice/Option, Tricare (all), WellCare PFFS, and Windsor Medciare Extra Plans.  If you choose to see Dr. Cross, you will be considered a self-pay patient. We will be happy to assist you in filing your own claim with your insurance company after services are rendered.

All patients with insurance need to present eligible medical insurance cards. We will file your insurance claims for you, but please understand that your health insurance is a contract between you and your insurance company. We will be happy to file for you, obtain pre-treatment estimates, and be as helpful as we can to help you obtain insurance payment. However, it is your responsibility to see that our fees are paid in full in a reasonable amount of time. The amount your insurance pays is based on the policy you bought and the amount of premium you pay. It is not based on our fees. We must emphasize as a medical care provider, our relationship is with you, not your insurance company. While the filing of insurance is a courtesy that we extend to you, all charges are your responsibility from the date the services are rendered.


The patient is responsible for making sure they bring their referral form to their appointment.  This form must be from the Primary Care Practitioner, unless otherwise specified. Check with your insurance provider to verify whether a referral is required for a specialist physician visit.

Self-Pay Policy

Payment is expected in full on the day of your appointment. A 25% discount will be offered to patients who pay in full at the time services are rendered. All payment arrangements must be authorized through our office prior to your appointment.
