Clinic PoliciesAppointmentsWe request that you notify the office at least 24 hours in advance if you will be unable to keep an appointment. We understand that your time is valuable to you and we will always attempt to see you promptly and at the scheduled time. Please assist us by being on time for your appointments. Should an emergency occur which requires a change in the schedule, we will notify you in advance if possible On your first visit, we will begin a permanent record. Please print off and complete the Patient Information Sheet and History & Physical Forms located on our homepage, prior to your appointment and bring them with you.PaymentThe financial responsibility for services rendered rests totally with the patient or responsible party, regardless of insurance coverage. Payment is due the day services are rendered. If for some reason you are unable to meet the financial requirements, we will be happy to work out a payment schedule with you prior to your appointment or scheduled procedure.InsuranceAll patients must present eligible medical insurance cards. We will file your insurance claims for you, but please understand that your health insurance is a contract between you and your insurance company. We will be happy to file for you, obtain pre-treatment estimates, and be as helpful as we can to help you obtain insurance payment. However, it is your responsibility to see that our fees are paid in full in a reasonable amount of time. The amount your insurance pays is based on the policy you bought and the amount of premium you pay. It is not based on our fees. We must emphasize as a medical care provider, our relationship is with you, not your insurance company. While the filing of insurance is a courtesy that we extend to you, all charges are your responsibility from the date the services are rendered.Due to an increased number of patients who are not paying their co-pays & deductibles, BTA will collect all co-pays before services are rendered and deductibles the same day of service. Per your insurance contract, you are responsible for these fees prior to payment of your claim by your carrier. ReferralsPatients with the following insurance are required to present their referral form at the time of service: Health Advantage, Qualchoice, Medicaid, Cigna-HMO, Emerson Electric-Healthscope, and Aetna-POS. Referrals must be from the primary care physician unless otherwise specified.All Champus/Tricare PatientsBTA DOES NOT accept Champus/Tricare as a primary insurance. We will continue to provide services to those patients who have Champus/Tricare as secondary coverage. However, all deductibles, co-insurance and co-pays will be collected at the time of service, based on your primary benefits.Self-Pay PolicyPayment is expected in full on the day of your appointment. All payment arrangements must be authorized through our office prior to your appointment.Your cooperation in the above will aid our staff in giving you the best service possible.
